As a MUSIC & MEMORY® Certified Care Organization, you are eligible to receive a free wall plaque from Music & Memory to showcase your certification. Follow these simple steps:

1. Log in to the Care Community and click on the name of your organization in the top navigation.
2. Navigate to the Obtain Plaque section of the page
3. Follow the instructions for filling out the required information and inserting the discount code for a free plaque, then submit the form.

Plaques typically arrive 1-2 weeks after ordering. Additional plaques are available at full cost.
